Clinic Policies

Clear information to help appointments, documents and requests run smoothly.

We kindly request your attention to the following essential policies and procedures.

School & Work Documents

Please submit forms or documents for the doctor at least 72 hours before the required deadline. Some forms may take longer to process, so please confirm the expected time with reception. We recommend booking an appointment when dropping off or collecting documents. Certain services or forms may involve a fee because they may not be covered by OHIP.

Prescriptions

For prescription refills, please book an appointment by phone or in person to consult with the doctor. Requests sent directly to the pharmacy without an appointment may include an administrative fee. For urgent medication needs, contact the clinic and allow up to three business days for processing. Reception can confirm the latest policies for specific prescriptions.

Appointment Times

While we aim to stay on schedule, unforeseen delays may occasionally occur. To help us manage appointments efficiently, please address one main concern per visit because standard appointments are limited in time. Arrive on time, as late arrivals may require rescheduling, and tell the front desk your appointment reason in advance. A fee may apply for missed or late appointments.

Lateness & No-Shows

If you are unable to attend, please inform the clinic at least 24 hours in advance. Patients arriving more than 10 minutes late may need to wait for the next available appointment. Missed appointments or late cancellations may result in a fee. Please contact reception with any questions.

Annual Physicals & Wellness Checks

The College of Family Physicians recommends wellness checks at least once a year, focusing on screenings such as Pap tests, mammograms and colon cancer screening, as well as vaccinations. You can book an appointment at any time to discuss current health concerns. Please refer to Choosing Wisely information on annual physicals for additional guidance.

Doctors’ Notes

Please see a doctor as soon as possible on the first day of your illness, especially if you anticipate that your employer will require a sick note. We will not provide a sick note to cover days of illness that cannot be evidenced.
